Use the sections below to view a number of forms produced for use by local governments.

Statutory forms

The following forms and certificates have been approved by the Director-General of the Department for use under the relevant provisions of the Local Government Act 2009 and the City of Brisbane Act 2010 and associated regulations. To meet statutory requirements, local governments must use these documents where required.

    Registers of interests

    provide transparency in local government decision making by disclosing details on a wide range of financial and non-financial interests of councillors, chief executive officers, senior executive employees of councils and persons closely related to these persons. The requirements relating to registers of interest are set out under the City of Brisbane Regulation 2012 (CoBR) and must be submitted by relevant persons, to the council's chief executive officer or mayor as required, in the following approved forms.

    Orders to enter property

    may be sought from a magistrate’s court to enable access to property for the purpose of undertaking specific works where an owner or occupier has denied access to council employees or their agents. The requirements for applying for an order and executing an order are set out under section 136 of the City of Brisbane Act 2010. An application for an order must be submitted to a magistrate using the following approved form.

    Warrants to enter property

    may be sought from a magistrate’s court to enable access to property where there are reasonable grounds to suspect that evidence of an offence under a local government related law may be found. Only an authorised person may apply for a warrant. The requirements for applying for a warrant and executing a warrant are set out under sections 119 and 120 of the City of Brisbane Act 2010. An application for a warrant must be submitted to a magistrate using the following approved form.
